Thank you for your interest in Oak Tree Medical Center. Our main objective is to offer you the most accurate diagnosis and correct treatment plan as efficiently as possible. There are two ways to become an Oak Tree patient:

CONSULTATION
We see patients in consult for spine conditions, minimally invasive procedures and fee-for service medical image review. After you consultation we will contact your physician of choice, obtain the correct prescription, communicate the results and offer recommendations. We do not prescribe medications.

BY REFFERAL
We are principally by referral only. Your physician may choose to elect our services or you can download our Referral Sheet here and request our services. Our goal is to work in conjunction with your referring physician(s) to obtain the precise diagnosis, best outcome or long term approach to your condition. After your doctor orders your consultation or appointment, we will contact your referring doctor, communicate the results and may offer recommendations.

Returning Patients
Welcome back! You know the drill. Please call out office to schedule a consultation or fax your doctor’s request to the fax number above and we’ll do the rest. Please take note that if you have moved, changed your name or have a new insurance; please fill out a new “Patient Information” sheet.

New Patients
First, you need to contact our office so we can assist you in scheduling either a consultation or appointment. Once you’ve decided on a service date, we will ask you to fill out some paperwork depending on your appointment type. Please click HERE to find our paperwork online.

Spine patients
At Oak Tree Medical Center, we perform both CT and C-Arm fluoroscopic guided cervical, thoracic, and lumbar selective procedures which are both diagnostic (to identify the exact source of pain) and therapeutic (to alleviate the pain). We do not, however, perform major spine surgery. We have a conservative management philosophy; first accurately diagnose the problem, then treat the problem and restore function while the body heals itself. Some cases will require major/minor surgery, if that time occurs we will be happy to work with you to find the best option for your care.

Imaging Patients
Oak Tree Medical Center is a radiologist owned and operated facility. We have on-site radiologist to review your exams and answer any questions you may have. Please call our office to schedule an appointment or fill out our online form.

Interventional Patients
Interventional Radiology is a sub-specialty of radiology which uses imaging-guided minimally invasive techniques to diagnose and treat a large variety of medical conditions that offers an alternative to the surgical treatment of many conditions and can even eliminate the need for hospitalization, in some cases. Interventional Radiology procedures involve the precise insertion of small specialized devices inside the body (such as catheters or guidewires) through tiny skin incisions using fluoroscopy, ultrasound, CT or MRI for guidance. Some of our procedures include:

• Vascular Access
• Varicose Vein Ablations

GENERAL INSTRUCTIONS

What do I bring to the first appointment?
Please arrive 20 minutes early so we are able to complete paperwork with you and obtain financial information. Bring your insurance card (please bring all cards, if you have more than one plan). If you have had recent imaging studies, please bring copies of the reports and images on a CD if you can.

Who obtains my authorization?
We will do our best to perform authorization services on your behalf; however, there are times in which we need referring doctor participation and even continuing patient participation. Ultimately authorization is an individual’s responsibility, so prior to your examination; please make sure your insurance has authorized all services.

What is your cancelation policy?
If you are unable to attend a scheduled appointment, you MUST inform the office at least 24 hours in advance. Of course emergencies and extreme situations will be considered. Our voice mail system is accessible for messages when the office is closed.
